The new model will ensure common functional goals among providers and standardized treatment outcomes to better support rehabilitation success.The intent of our Rehabilitation Program is to provide effective Veteran-centered care that responds to the medical, psycho-social and vocational rehabilitation needs of eligible Canadian Forces Veterans. The goal is to help reduce barriers in relation to a participant’s eligible health problem(s) and to help adjust to life at home, in community or at work.
