Clinical Nurse Specialist, Primary Care Salary Range: CAD $54.19/hr – CAD $77.89/hr Vancouver Coastal Health (VCH) is seeking a Clinical Nurse Specialist (CNS) to join the Primary Care Team. The CNS will support Team‑Based Care for equity‑seeking populations across Vancouver Primary Care and will play a key role in the newly formed Groundspring Primary Care Team, Three Bridges, Ravensong, Downtown Community Health Centre, and SHeway. Responsibilities Report operationally to the Vancouver Operations Director, Primary Care, and provide professional accountability to the VCH Vice President Professional Practice. Lead and direct nursing care delivery, formulate organizational policies, and foster high standards of service delivery. Provide expert advice on nursing, resource planning, program and health system improvement, research, and professional development to support quality and utilization initiatives. Continuously evaluate practice, evaluate research, and develop frameworks to facilitate the implementation of best practices. Serve as a clinical leader in Primary Care, advance nursing practice through evidence‑based research and advanced practice. Act as liaison between hospitals and community programs to integrate care across the continuum and report on professional nursing practice matters. Lead and consult with team members, healthcare staff, patients/clients, families/significant others; collaborate with Program Managers, Medical Directors, Clinical Educators, Nurse Clinicians, and interdisciplinary team. Proactively move nursing practice forward at the regional, provincial, and national levels. Education & Experience Master’s degree in Nursing and current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Minimum of five (5) years of recent experience in Primary Care with a progressive leadership role. Canadian Nurses Association certification in Primary Care or membership in related professional associations preferred. Knowledge & Abilities Collaborates to build relationships that promote cooperative goals and maintain trust and mutual respect. Maintains strong relationships with internal and external partners to enhance trauma‑informed, culturally safe care. Provides professional consultation to share expertise with caregivers, patients/clients, families/significant others, and colleagues. Delivers education and orientation on advanced practices, ensuring skill competency. Utilizes acute clinical evaluation skills to assess patient needs. Identifies and develops discipline‑specific standards for teaching and education in partnership with academic institutions. Leads research initiatives and informs planning, implementation, and evaluation of activities. Fosters a motivating work environment, respectful relationships, and decision‑making support. Demonstrates advanced verbal, written, and technological communication for disseminating information and research. Applies cultural diversity knowledge in a health care setting. The hours of work, including days off and work area, may be subject to change consistent with operational requirements and the applicable statutes.
