Respiratory Practitioner (Nurse / Physiotherapist) The Oxford Health NHS Foundation Trust Respiratory Team deliver 4 aspects to their service - Community, hospital in-reach, home oxygen service and pulmonary rehabilitation. We are looking for a passionate and dedicated Nurse or physiotherapist to play a key role in delivering high quality patient centred respiratory care within the community. The role will initially be for the community and in reach service however integration will be expected into the other arms of the service as per service needs. The post holder will will across patients homes, community clinics, in reach to a hospital setting, collaborating closely with GPs, acute services and wider community teams. You should have a minimum of 2 years experience since qualifying and ideally some of this within respiratory but will consider transferable skills. The post holder will contribute to proactive care, admission avoidance, supported discharge and self management education, using evidence based practise and national/local respiratory guidelines. You will also play a role in service development, audit and quality improvement. Main duties of the job Provide specialist respiratory assessment, treatment and ongoing management for patients in a community setting. To manage patients during acute exacerbations of their condition, giving advice and guidance when patients are in a hospital setting and working alongside community and primary care to manage the patients in the community until patients are stable and able to self-manage. Promote Safeguarding, equality, diversity and patient safety in all aspects of care. Undertake comprehensive clinical assessments, interpret relevant investigations and formulate evidence based care planning. Provide education and self management support to patients and their carers, including inhaler technique, exacerbation management, smoking cessation and lifestyle interventions. About us Team members are supported by experienced senior team members and are expected to support across all aspects of the service. The team have regular clinical support, and regular education sessions and always have access to senior advice. There is a strong working relationship with secondary care colleagues who are also available for advice. Our primary aim is to promote client/patient self-management at home and prevent hospital admissions. This is delivered via an in-reach hospital service, community /Home Oxygen assessment and review service and a countywide Pulmonary Rehabilitation Service. All applicants will have excellent communication skills and be able to work autonomously, seeking support as required. The current service hours are Monday - Friday 08.30-16.30. Please note that these hours maybe reviewed in the future. Permanent and fixed-term contracts available. Please note: New starters have a 6 month probationary period. This, together with the induction process aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale. Candidates not currently employed by the Trust who attend an interview for non-qualified Band 1-5 posts are required to undertake numeracy and literacy assessments. Person Specification Qualifications Registered General Nurse (with current registration with NMC) and educated to degree level; or Diploma / BsC in Physiotherapy with current HPC registration. Non-Medical prescribing Knowledge Knowledge and understanding of a range of respiratory conditions, particularly airways diseases Ability to demonstrate advanced clinical skills with particular reference to managing respiratory patients Willingness to further develop respiratory skills Evidence of personal and professional development and of reflective practice e.g. CPD portfolio Experience Experience of working with respiratory patients Experience of teaching in small group situations Experience of working within a community setting Experience of managing palliative and end of life care Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
