Detailed Overview Reporting clinically to the Professional Practice Chief, Social Work and administratively to the Manager, Clinical Operations, the Social Worker will provide comprehensive services and therapeutic clinical counseling to patients and families for the assigned clinical specialty area; conduct comprehensive social work and psychosocial assessments using a variety of tools; educate and consult patients, families, and other health care professionals; participate in committees and planning bodies; develop and deliver in‑services, presentations, psycho‑educational and support groups; and initiate, develop, and carry out research activities. Responsibilities Conduct comprehensive social work and psychosocial assessments of patients and families. Provide therapeutic clinical services such as grief and bereavement counseling. Consult and educate patients, families, and health professionals on high‑risk factors, recovery, and current practices. Maintain patient records, charting, progress notes, and reports in compliance with standards and regulations. Lead interdisciplinary team in patient care planning, discharge planning, and hospital stay care plans. Arrange admissions meetings and coordinate community services for device placement and home support. Communicate with community health agencies, provincial ministries, and family physicians to manage referrals and resources. Act as a clinical resource for community resources and information. Participate in committees and planning bodies to evaluate programs, identify improvements, and enhance service delivery. Maintain workload records, analyze trends, prepare reports, and recommend action plans. Develop, coordinate, and deliver in‑services, presentations, psycho‑educational and support groups. Initiate, develop and conduct research activities and program planning aligned with patient, family, and community needs. Continuously develop professional knowledge and engage in professional development activities. Perform other related duties as assigned. Education and Experience Master’s degree in Social Work from an approved school of Social Work and at least one (1) year of recent related acute care clinical experience. Current full registration with the BC College of Social Workers. Applicants with a Bachelor of Social Work (BSW) degree from an accredited School of Social Work, and a demonstrated equivalent combination of education, training and experience may be considered.
