The company is recognized as one of Canada’s Best Managed Companies. Job Description You will be responsible for delivering high‑quality nursing care to patients in the community. Your clinical skills and deep knowledge of wound, ostomy and continence care management will support patients with chronic diseases. Responsibilities Conduct comprehensive assessments and develop individualized care plans for patients with wound, ostomy, and continence concerns. Provide patient‑focused, safe, holistic nursing care and work collaboratively with team members. Ensure nursing care is based on current best practices to achieve successful outcomes. Continually assess, anticipate, identify and respond to new and changing patient needs. Travel between patients’ homes within a specific geographical region. Qualifications Completion of NSWOC or International Interprofessional Wound Care Course (IIWCC). Registered and in good standing with the College of Nurses of Ontario (CNO). Minimum of two years nursing experience; community nursing experience is ideal. Ability to work independently managing your own caseload. Proficiency in English, both verbal and written; additional languages a plus. Valid Ontario driver’s licence and reliable vehicle with willingness to travel. Basic proficiency in Word, Excel and ability to learn software.
