Clinical Pharmacist – BC Children’s & Women’s Hospital (Vancouver, BC) Fraser Health Authority – Relief Full Time (until September 2026 or return of incumbent) Salary CAD $51.37 – $64.19 per hour Job Summary We are seeking a Clinical Pharmacist to provide individualized pharmacotherapy, patient counseling, collaborate with physicians and other health care providers, develop drug therapy guidelines, contribute to publications, serve as departmental resource, participate in quality improvement and clinical drug trials, and mentor pharmacy students and residents. Responsibilities Consult with physicians, nurses and other health care providers to develop therapeutic plans and ensure documentation in the patient health record. Individualize patient medication therapy by reviewing literature, patient charts, lab results, and assessing clinical response. Provide patient medication counseling, demonstrating procedures and assessing understanding. Assist in developing drug therapy guidelines and monitoring effectiveness. Participate in ward rounds, program team meetings and unit-level pharmacy services. Act as department resource person; support staff with drug information. Contribute case studies and review articles to professional journals. Provide instruction and/or supervision to pharmacy students and residents. Participate in quality improvement activities, audits, and recommendations. Perform drug distribution duties, interpret prescriptions, dispense medications using computerized system. Review prescriptions for therapeutic suitability and communicate clarifications. Ensure accurate dispensing and labeling, maintain required records. Participate in medication use evaluation projects and maintain inventories. Participate in clinical drug trials and adverse reaction documentation. Implement medication safety initiatives and quality control activities. Attend staff meetings, committees, and identify opportunities to improve processes. Maintain records for narcotic and controlled drugs, special access program drugs, and departmental statistics. Prepare reports on activities, maintain spreadsheets and documents. Perform other related duties as assigned. Qualifications Education and Experience Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or equivalent. Skills And Abilities Broad knowledge of organization policies, procedures and standards of care. Knowledge of evidence-based clinical pharmacy practice, medication use evaluation, research processes. Knowledge of drug distribution processes in a hospital setting. Ability to apply clinical pharmacy knowledge and make decisions with minimal supervision. Effective verbal and written communication. Establish and maintain effective working relationships with health practitioners, patients, families and public. Organize and prioritize work. Operate relevant equipment. Physical ability to carry out duties of the position.
