Job Details Job Number: JR104228 Job Title: Case Manager, Crisis - Psycho Geriatrics Resource Team Job Category: Paramedical Hospital Location: General Site Job Type: Temporary (Fixed Term), Full time Contract Duration: 11 months Number of Positions: 1 Union: OPSEU Minimum - Maximum Hourly Rate: $44.24 - $53.69 Hours: All Shifts Position Overview The Psychogeriatric Resource Consultation Program (PRCP) in the Greater Toronto Area (GTA) provides Knowledge-to-practice (KTP) services to individuals in long-term care homes and community services agencies caring for: senors suffering from behavioural disorders associated with a dementing illness and other mental and neurological illness’ vulnerable adults who may be less than 65 years of age but whose cognitive and functional abilities are compromised by early onset dementia, developmental disability or acquired brain injury. The Psychogeriatric Resource Consultant (PRC) will be an employee of a host organization and liaise with appropriate ‘in‑house’ services and local psychogeriatric outreach and Behavioural Supports Ontario services. As well, the consultant will join a team of 12 other PRCs distributed across the region and report to the PRC Program in a distributed matrix reporting model. While based at the host organization, the consultant’s work will be focused in long-term care homes and community service agencies in an assigned catchment area. The PRC will develop a needs-based area of special interest and serve as a resource in this area(s) across the GTA. The PRC will be required to provide services in accordance with the PRCP’s KTP framework leading to practice and workplace improvements within the serviced organizations and enhanced services integration. The PRC will optimize opportunities to collaborate with other PRCs, demonstrate leadership in the care of the target population and participate in the program’s accountability initiatives. Each PRC is responsible to adjust her/his work schedule to meet the needs of their customers. Requirements A degree and/or certification in one of the following disciplines – nursing, social work, rehabilitation, psychology or gerontology At least 2 years’ experience in psychogeriatric services is preferred, otherwise significant experience in geriatrics, long-term or community-based seniors care is essential Demonstrated competence with standard psychogeriatric assessment tools Knowledge and experience in adult education Proficiency in using computing software such as Word, PowerPoint, Email and the Internet Driver’s license and ability to travel across the region and the GTA on a daily basis Demonstrates a commitment to customer service when dealing with patients, colleagues and volunteers and other members of the community and public Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation Demonstrates an understanding of and commitment to SHN's comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area Demonstrates excellent work performance and attendance record Assets The ability to converse in a second language Experience with standardized curricula such as P.I.E.C.E.S., U-First, Gentle Persuasive Approach Knowledge/experience in the knowledge to practice process Accommodation And Diversity Statement Scarborough Health Network (SHN) embraces and celebrates our community’s unique multicultural heritage and diversity.
