Detailed Overview As a member of the designated community Mental Health & Substance Use Services or Integrated Response team, you will support engagement, care coordination and system navigation across substance use & mental health services, local service areas, and regional and provincial services, with an emphasis on reducing barriers to enter/re-enter the system of care and facilitating timely treatment and rapid re‑engagement of clients where appropriate and feasible. You will provide client and family‑centered care through a trauma‑informed approach and evidence‑informed practice to individuals and families affected by serious and persistent mental disorders and alcohol and drug addictions. In a shared‑team model, you will provide initial screening, intake, client assessment, treatment recommendations and plans, determine referral needs, follow‑up support, provide therapy including group therapy, liaise regarding complex situations, provide group education, and counsel clients. You may also provide outreach as required and, if assigned to Aftercare services, be the “most responsible clinician” for specific clients depending on their primary needs. You will maintain respect for client dignity and self‑determination, participate in local committees and internal and external case conferences, develop and deliver in‑services, presentations and training, and contribute to data analytics and process improvement activities. May be required to work at more than one location and/or on an outreach basis to assess and engage clients within their own environment and community. Responsibilities Perform initial screening of referrals to determine appropriate treatment based on client needs, urgency, and best practices. Assess client needs by gathering client information via phone or other methods, collaborating with the interdisciplinary team, and determining the need for emergent crisis intervention. Provide first level intervention and support until formal treatment or further assessment can be conducted. Provide treatment/support and case management for complex situations, including delivery of individual and group therapy. Provide case management, individual counseling, and brokered connection to community services for longitudinal aftercare services. Facilitate client utilization of appropriate community services and rehabilitative programs by researching available services and referring as required. Provide referrals to appropriate community treatment alternatives and services. Provide education to clients and families in evidence‑based practice and relapse prevention strategies. Provide short‑term counseling to clients and families and information to health providers and the general public. Maintain client records using PARIS or other clinical information systems. Ensure compliance with health and safety requirements and report unsafe situations. Develop linkages with other community resources and advocate on behalf of clients. Maintain relevant data and statistics on intakes, referrals, and provide input to the coordinator and manager. Promote quality improvement through recommendations on policies, processes, and procedures. Participate in team planning, meetings, committees, and corporate initiatives. Maintain current knowledge of mental health and substance practice through professional development. Perform other related duties as assigned. Qualifications Education, Training and Experience Master’s Degree in an applicable social or health sciences program such as Counselling Psychology or Social Work. Two (2) years’ recent related experience working with clients and families affected by mental illness and substance use disorders, or an equivalent combination of education, training and experience. Eligible for registration with relevant professional association. Valid B.C. Driver’s License and access to a personal vehicle for work‑related purposes. Skills and Abilities Demonstrated knowledge of harm reduction, trauma‑informed, recovery‑oriented approach; concurrent disorders; addictions counselling. Comprehensive knowledge of relevant Acts such as Mental Health Act, Freedom of Information Act, and pertinent regulations. Comprehensive knowledge of psychiatric diagnosis and professional practice guidelines for the treatment of various disorders. Ability to utilize mental health and addictions standardized tools for client assessments. In‑depth knowledge of community resources and strong collaborative skills. Effective verbal and written communication skills in a diverse, multicultural community. Ability to work effectively with interdisciplinary professionals. Strong organizational, planning and problem‑solving skills in a complex environment. Ability to develop and deliver in‑services, education, presentations and support groups. Therapeutic skills in case management, individual and group therapy with adults with various mental disorders. Proficiency with personal computers and related software. Physical ability to carry out the duties of the position.
