Advanced Practice Leader, Wound Care Program Baycrest is seeking an advanced practice leader in the Wound Care Program to improve patient outcomes. The role reports to the Manager and the Interprofessional Practice Chief for Nursing and works with interprofessional teams across Baycrest Hospital and Apotex, Baycrest’s Long‑Term Care Home. It focuses on patient and caregiver education, strategic program development, and scholarly gerontological care. Responsibilities Lead design, implementation, and continuous enhancement of evidence‑based wound, ostomy, and continence care program aligning with organizational goals. Provide advanced clinical consultation and guide managing complex cases for staff, patients, residents, and families. Develop and deliver educational initiatives to build team capacity and sustain improvements. Create, review, and maintain policies and procedures ensuring safe, consistent, and regulatory‑compliant care. Champion prevention strategies to reduce risk of pressure injuries and peristomal complications. Drive quality improvement initiatives focused on wound, ostomy, and continence care outcomes. Perform debridement and advanced wound care procedures as needed. Lead evidence‑based practice development and quality initiatives promoting high‑quality patient‑centered care. Collaborate with interprofessional teams and act as change agent to implement best practices. Advocate for patients/residents and families regarding therapeutic interventions and healthcare access. Engage in evidence‑based practice by critically appraising and applying current evidence. Lead or support research initiatives to integrate evidence into clinical practice. Provide leadership in managing clinical care and integrate the wound care APL role within Baycrest community. Monitor program outcomes using metrics, provide feedback, and report to highlight achievements. Act as clinical on‑call resource on a rotational basis. Contribute to nursing profession development through active involvement with professional associations. Qualifications Registered Nurse (RN) with the College of Nurses of Ontario in good standing. Master’s degree preferred. Specialty certificate or completion of wound care education programs (e.g., Wound, Ostomy and Continence CNA Certification, International Interprofessional Wound Care Course). Experience in advanced wound debridement (asset). Minimum of five years clinical geriatric bedside experience in a hospital setting. Minimum of two years wound care management experience. Current Basic Cardiac Life Support (BCLS). Excellent clinical skills and competencies. Experience teaching, precepting or working with clinical learners. Superior verbal, written, listening, and communication skills. Collaborative working skills and ability to work with all levels. Strong organizational, planning and time management skills. Ability to manage competing priorities and critical thinking. Demonstrated leadership, change management and problem‑solving capabilities. Compensation Salary range: $102,072 to $127,590 annually (target range: $102,072 to $120,000). Salary will be determined based on experience, skills and market conditions ensuring fairness and internal equity. Benefits Vacation entitlement.
