As a Health Care Connector, you will be responsible for navigating patients through a complex health care system by connecting people to an appropriate primary care provider (Unattached Patient Program – UPP). Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient‑centred care – and be supported by our collaborative team that includes over 8,900 regulated health care and other professionals. Projected start date February 23, 2026 with mandatory orientation. Home branch Burlington. What will you do? Work collaboratively with patients and others in their support network to gather information to make a successful connection with an appropriate primary care provider based on the patients’ care needs. Maintain contact with patients on the Ontario Health atHome priority list to keep them informed about prospects for attachment. Work with patients to help them understand and manage their short and long term health care goals. Provide information about the Unattached Patients Program, patient rights and responsibilities, and the primary care services available. Provide information and referral assistance to patients to access alternative community resources (walk‑in clinics, after‑hours clinics, urgent care centres, etc.). Support patients who face a longer wait time in continuing their efforts to find an appropriate provider during the attachment process. Obtain consent for the gathering and sharing of patient information acquired through the attachment process. Evaluate patients’ satisfaction to identify opportunities for improvement as well as trends (e.g. identify geographical areas experiencing the greatest access problems). Use sound professional judgement and act in accordance with all Ontario Health atHome policies and procedures. Establish and maintain effective relationships with Ontario Health atHome, local agencies (hospitals, family health teams), primary health care professionals, and other appropriate agencies to build and maintain an inventory of primary health care professionals willing to accept new patients. Represent Ontario Health atHome on multi‑disciplinary committees and community working groups. Participate in events to promote Ontario Health atHome objectives, activities, programs and services. Share knowledge, requests information and collaborate with superiors and colleagues to deliver effective and efficient patient service delivery. Provide front‑line facilitation to primary care providers and is responsible to make decisions and take action within legislative framework, Ontario Health atHome protocols, policies, and procedures and best practices quality patient care. Exercise a high degree of patient sensitivity, caution and sound professional judgment to promote a positive image of Ontario Health atHome to the community, enhance public confidence in Ontario Health atHome's services to and role in the community, and mitigate against legal and financial risks/liabilities. What must you have? Registered Practical Nurse (RPN) with current registration with the College of Nurses. Good knowledge of primary care services and other community resources in Ontario Health atHome, as well as the roles of health care professionals. Practical experience in diverse and unionised community health care environments. Understanding of issues and priorities within the health care sector. Specialised knowledge regarding specialty programme(s) assigned. Practical knowledge and understanding of relevant legislation. Effective planning, organising and evaluation skills to manage multiple patients, provide information reports and take corrective action as required. Ability to use MS Office applications (Word, Excel, Power Point). Strong interpersonal skills to work with a diverse patient groups with varying levels of comprehension and language capability. Effective communication skills to establish and maintain a wide range of contacts with professionals and organisations within the community. Strong collaboration skills to participate on projects and committees with colleagues across Ontario Health atHome and with local community agencies and health professionals (regarding patient care and Ontario Health atHome patient care services). Effective listening and strong facilitation skills are necessary to work with patients and their families in order to ensure best possible attachment is provided to the patient. Ability to communicate using various software programmes to clearly articulate patients’ status and service needs. Ability to deliver information effectively in a variety of settings including one‑on‑one, team meetings, and presentations. As the front line facilitator to primary care providers, this position is required to resolve issues regarding the appropriate attachment between patient and provider and must consider a broad range of elements related to patient needs and community resources. Uses information from patient, patient’s family and providers to formulate recommendations. Refers to known resources, established standards and precedents, legislation, policies and procedures to develop solutions to meet patient needs; when no established precedents exist, creativity/innovation may be required to deal with needs/requirements unique to the patient.
