Supported Employment and Education Specialist/Case Manager Will work in conjunction with a multidisciplinary team in the role of case manager to remove barriers to treatment participation and access community resources and as the SEES create and implement individualized “Education/Vocational Plan” including identifying interests, strengths, skills and challenges and barriers to education and employment. Education, Knowledge, Skills & Experience Required Bachelor's degree in counseling, human services psychology or related field, plus one to two years related experience and/or training. Experience in supported employment as an evidence-based practice preferred. Current Colorado Driver’s license and DMV background clearance to transport clients. Must have the ability to engage persons with severe/persistent mental illness to address case management needs, employment and education goals. Case Management Duties Organizes, plans and coordinates routine client outings to promote social skills, reduce isolation and build trust with the treatment team staff. Outings may require transportation of clients to and from events. Resource acquisition including assisting clients with application for benefits, basic needs such as housing, food, clothing, medical, dental care, legal, vocational, psychiatric services, etc. Co-facilitate groups as needed and on a schedule determined by the Ascent Coordinator. Coordinates with internal and external providers to assist clients in reaching their goals. Engage client and family in the treatment process and goal setting. Outreach clients and effectively engage clients in supportive services. Must be able to provide needs assessments regularly for families and link clients to the relevant services. Work with clients will regularly take place in the community and/or clients home. Effectively plans for encounters to ensure that clients get the maximum benefit from each service provided. Works to increase and maintain client self-sufficiency by increasing client’s adaptive level of functioning and use of community and natural support. Maintains appropriate professional standards and provides appropriate follow-up for consumers. Exhibit very strong communication with treatment teams involved in a case. Works collaboratively with treatment teams to address barriers to treatment. Attend and effectively participate in meetings to advocate for the needs of clients. SEES Duties Provide support for education goals in the community, including contact with staff at high schools, colleges and other educational institutions. Providing individualized support to assist clients in maintaining employment or education. Provide employment support from assessment through long-term follow-up when a client obtains employment. These job development services include but are not limited to assessing skills and strengths, identifying target jobs, teaching job seeking skills, supporting resume and cover letter development, providing job coaching and training. Will provide ongoing career planning to support clients’ long-term goals. Engaging clients and establishing trusting, collaborative relationships directed toward the goal of education or competitive employment in community job settings. Work with businesses in the community to establish professional relationships that will assist members in their recovery. These include but are not limited to the development of employment opportunities, transitional employment opportunities, job coaching, and increased employer awareness of the role of the Ascent team in the community. Participate in all meetings and trainings relevant to the Vocational Services Program and Jefferson Center. Other Duties Comply with all Jefferson Center policies and procedures including those of confidentiality, conduct, and record keeping. In addition, Support clients in appropriately advocating for themselves. Maintain a valid Colorado Driver’s License, current vehicle insurance, and reliable transportation. Provide transportation for clients when appropriate and as directed. Provide accurate, thorough documentation in compliance with guidelines, deadlines for Jefferson Center and other funding sources. Maintain effective interpersonal relations with clients, peers, subordinates, upper management, visitors, and the public. Collect and provide client statistics for quarterly data submitting to Signal and the BHA Reinforces treatment team modalities to clients on symptom management using strengths-based approaches. Submit 90% of PTD forms within 3 working days. Identify risk factors and coordinate with Ascent clinical staff or the crisis team for next steps. Promotes client engagement in treatment with all Ascent staff. Exhibits enthusiasm, respect, adaptability, flexibility, and spirit of cooperation in the work environment. Maintains appropriate professional standards and provides appropriate follow-up for Demonstrates ability to work with clients with diverse backgrounds. 24 business hour response time to phone calls, e-mails, texts and other requests. Attend mandatory in-service training, and compliance with individualized training plan if required. Participate in supervision by coming prepared with an agenda. Report high risk/problem cases and utilize a problem-solving approach as well as receiving feedback. Attend supervision at times and intervals agreed upon with supervisor. Meet required productivity service hours as determined by Ascent Coordinator. Note: Employees are held accountable for all duties of this job.
