Public Health Nurse – Bilingual (Job ID 31356) Join the City of Hamilton’s Public Health Department as a bilingual (French/English) nurse to support the health and wellbeing of children and youth in school communities and within the broader population. This role works in partnership with health professionals, community partners, and school staff to promote health equity and manage public health responses. Job Summary Public Health Nurses (PHN) integrate knowledge from public health sciences, primary health care, nursing science, and social sciences to promote, protect, and preserve community health. PHNs use clinical expertise and social context to deliver public health nursing practice in a variety of settings, addressing health equity and positive population health outcomes. PHNs perform duties under the direction of the Medical Officer of Health and in compliance with relevant legislation and City policies. Responsibilities Assessment & Planning Access high quality data and research to plan evidence‑informed programs and interventions. Apply population health assessment data to identify health concerns and priorities. Collaborate with clients to conduct comprehensive needs assessments. Develop mutually agreed care plans that consider health equity, trauma‑informed care, and cultural safety. Plan services using a wide range of promotion strategies including reorientation of health services, supportive environments, healthy public policy, community capacity building, and individual skill development. Intervention Implement programs, services and interventions per best practices and policy. Implement medical directives authorized by the Program Manager and Medical Officer of Health. Collaborate with colleagues, health professionals and community partners to provide comprehensive client care. Provide nursing care using an asset‑based, client‑centered, trauma‑informed, harm‑reduction, cultural safety approach. Facilitate informed consent and shared decision‑making with clients. Use strategies that address health equity and socio‑economic determinants of health. Overcome inequities and facilitate access for vulnerable populations (e.g., outreach, home visits). Ensure seamless transitions to care and connect clients to appropriate programs. Build relationships with community partners to address health issues. Develop reports for the Program Manager. Respond to public health threats or emergencies, including redeployment if required. Evaluation Engage in critical reflection for continual improvement. Monitor outcomes and adjust care plans with clients. Participate in program and departmental evaluation and continuous quality improvement. Participate in research projects approved by the Program Manager. Contribute to budget reporting as requested. Professional Nursing Practice Contribute to team‑based service delivery and reduce barriers to community needs. Maintain timely communication with supervisors and team members. Document nursing activities per certification standards and City policies. Maintain professional boundaries in all relationships. Support, mentor and teach public health team members, including precepting students. Identify and address professional growth areas, seeking development opportunities. Stand by decisions that serve public health objectives even if unpopular. Behave in accordance with City conduct standards and respect the business agenda. Act as a City ambassador in external communications. Other Duties Work in a variety of settings: community spaces, schools, client homes, clinic, flexible office. May be required to work evenings and weekends. Intense auditory and visual concentration required. May need to lift, carry, and transport resources. May have prolonged periods of sitting. Qualifications Entitled by the College of Nurses of Ontario to practice as a Registered Nurse with current registration. Baccalaureate Degree in Nursing from an accredited program that includes public health nursing. If not, must be supplemented with a certificate or diploma in public health nursing obtained after at least one academic year. Fluent in French (written and verbal); must pass French competency assessment. Demonstrated knowledge of public health nursing competencies and standards. Preferred: Prior public health nursing experience. Demonstrated proficiency in core public health competencies, independent practice, entry‑to‑practice competencies, and therapeutic relationship building. Excellent communication, teamwork, leadership, and reflective practice skills. Proficiency in maintaining high performance standards. Provision of personal vehicle for work; Class G driver’s licence required. Other Requirements Personal vehicle required 3+ times per week; parking provided. Must provide immunization records, including TB testing, prior to start.
