In this role, you will have access to clinical leaders and supervisors, communication therapist assistants, best practice specialists and students to support the safe and joyful delivery of high‑quality, personalized care.Responsibilities (In‑Person or Virtual Delivery)Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home‑based settingUse of a collaborative and interdisciplinary team‑based approach to care and servicesSupport a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc.)Leverage our digital health platforms to support care delivery, collaboration, documentation, and interventionWhat You Need to Be SuccessfulRegistered as a member in good standing with the College of Audiologists and Speech Language Pathologists of OntarioValid driver’s license and vehicle insuranceFocus on person and family‑centered careComfortable delivering care both in person and using virtual methods/technologyPassionate, empathetic, use good judgement and apply critical thinking when making client decisionsExcellent communication skills and ability to work collaboratively within an interdisciplinary team as well as work independentlyFlexible and demonstrates skills in critical thinking to apply to a variety of situationsValues the role a therapist plays in keeping clients at home, where they want to beAbout CBI Home HealthIf you are passionate about offering a person‑centered approach to care, this is your chance to do what you do best.
