Overview Vancouver Coastal Health (VCH) is seeking a Social Worker – Case Manager to support clients with complex mental health, addiction, and chronic medical concerns in Squamish, BC. The role involves providing case management and clinical services, conducting assessments, coordinating care, and collaborating with an interdisciplinary team. Responsibilities Provide case management services and clinical and consultative social work services to individuals with complex mental health, addiction and chronic medical concerns. Conduct biopsychosocial mental health and addiction assessments, provide counselling services, maintain client records, and attend interdisciplinary clinical/case conferences. Identify applicable community resources, contribute to the development of community resources, and plan care and support in accordance with clients’ health needs. Ensure continuity of care across the continuum by liaising with team members, developing and evaluating care plans, and providing crisis intervention. Act as the Designated Responder under the Adult Guardianship Act, when appropriate. Coordinate and implement clinical interventions, evaluating client outcomes based on client-goals for care. Coordinate ongoing care and supports and transition to the community; consult, confer, and collaborate with interdisciplinary team members, physicians, educational resources, and other health care professionals to achieve excellence in client‑centred education, care and empowerment. Education & Experience Master’s degree in Social Work. Two (2) years’ recent experience in case management with individuals with complex mental health, addiction and chronic medical concerns, or an equivalent combination of education, training and experience. Current full registration with the British Columbia College of Social Workers. Local area travel requires the use of personal vehicle. Valid BC Driver’s License. Knowledge & Abilities Knowledge of recovery principles in mental health. Knowledge of Psychosocial Rehabilitation methodology. Knowledge of mental health issues and treatment, substance abuse and addiction treatment, and psychopharmacology (indications and side‑effects). Ability to apply evidence‑based practice case management principles within a client and family‑centred care model. Knowledge of Indigenous culturally competent and responsible care. Ability to conduct biopsychosocial mental health and addiction assessments, functional/environmental and risk assessments. Knowledge of systems thinking and the social work process. Demonstrated skills in mediation, conflict resolution, counselling, education, project planning, and teamwork. Knowledge of relevant statutes and regulations (e.g., Mental Health Act, Adult Guardianship Act, Health Care Consent Act, FIPPA). Experience in Harm Reduction and Trauma‑Informed Care. Fundamental computer literacy (client care system, word processing, spreadsheets, internet, email). Hours and Compensation The salary range for this position is CAD $42.27/Hr. – CAD $52.81/Hr. Hours of work, days off and locations may vary in accordance with operational requirements and the Collective Agreement.
