Responsibilities Assess and provide dietetic interventions to adults, geriatric and pediatric clients in their homes using evidence‑based best practice. Coordinate care through ongoing communication with the client care team, creating opportunities for more independence and promoting VHA’s core beliefs and values. Prepare client reports for health professionals, analyze treatment plans, and present thorough information to clients and caregivers. Educate internal and external staff about healthy eating and proper nutrition. Support interventions for failure to thrive, dysphagia, feeding problems, enteral feeding, diabetes, cancer, GI disorders, weight management, and palliative care. Help patients accept their illness and advise on special and time‑specific diets. Develop knowledge and skills in continuous quality improvement to support safe client care. Qualifications Master’s degree in Dietetics from a recognized school of Nutrition and Dietetics. Member in good standing with the College of Dietitians of Ontario. Minimum 2 years clinical experience (optimal). Valid driver’s license with access to a vehicle. Compassionate, empathetic and self‑driven with the ability to work independently.
