Clinical Pharmacist, Community Pharmacy Programs – Fraser Health Community Fraser Valley Apply now to join the Fraser Health Authority as a full‑time Clinical Pharmacist in the Community Pharmacy Programs at Abbotsford, BC. Salary CAD $51.37 – $64.19 per hour Job Summary Provide clinical pharmacy services to community‑dwelling clients, including individualized medication therapy, counselling, and collaboration with the health care team. Participate in quality improvement, medication safety initiatives, and academic detailing for physicians. Key Responsibilities Consult with physicians, nurses, and other health care providers to develop and discuss therapeutic plans appropriate for each client. Individualize client medication therapy, review charts and laboratory results, and adjust plans as needed. Provide patient medication counselling and education. Assist in developing drug therapy guidelines and review current literature. Participate in medication use evaluations, quality improvement, and safety initiatives. Provide academic detailing to optimize prescribing practices. Contribute case studies and review articles to professional journals. Supervise pharmacy students, residents, and new staff. Maintain accurate documentation in client health records in accordance with regulations. Assist with inventory management, requisition of drugs and supplies, and monitoring expiry dates. Prepare reports and documentation as required. Perform other related duties as assigned. Qualifications Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or equivalent combination of education, training, and experience. Valid BC Driver’s Licence and access to a personal vehicle for business purposes. Broad knowledge of organizational policies and evidence‑based clinical pharmacy practice. Knowledge of medication use evaluation, research methodology, and drug distribution processes in a hospital setting. Ability to apply clinical pharmacy knowledge independently and communicate effectively with patients and health care professionals. Strong organisational, prioritisation, and equipment operation skills. Physical ability to perform the duties of the position.
