Services you will provide Individualised pharmacotherapy Counselling patients about their medication therapy Work in collaboration with physicians and other health care providers to develop, implement and monitor therapeutic plans Assist in the development of drug therapy guidelines for individual disease conditions Prepare and submit case studies and articles for publication in professional journals Act as department resource person within your assigned area Participate in medication use evaluations, quality improvement initiatives and clinical drug trials Provide mentorship and supervision to pharmacy students, residents and new staff Ensure prescriptions are validated and dispensed accurately, and resolve medication safety concerns in collaboration with the health care team Document professional practice activities proficiently To join us you will need Current registration as a Pharmacist with the College of Pharmacists of British Columbia Completion of an accredited Hospital Pharmacy Residency Program Benefits we offer A career that offers you the opportunity to work in a fast‑growing and exciting organization with professionals who are eminent in their respective fields Competitive salary package, including comprehensive health benefits coverage Responsibilities Identify and resolve actual and potential drug therapy problems in collaboration with the client and other health care team members Provide comprehensive medication management and pharmaceutical care by monitoring medical conditions and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions, and therapeutic, chemical and laboratory incompatibilities; communicate findings, recommendations and plans to clients and healthcare practitioners Consult with clients and team members to develop client‑specific therapeutic plans based on chart review and clinical assessment Maintain appropriate documentation in the health record according to organisational standards and practices Use appropriate resources to support professional practice and client care, including those available through provincial programmes Establish linkages with health authority, community pharmacy services and other relevant community services to support continuity of care for clients Provide drug information, consultation and support services to medical, nursing, pharmacy and other health care team members Conduct client counselling and education regarding drug therapy Provide in‑service education to the health care team where appropriate Provide instruction and supervision to students such as University Pharmacy Undergraduates and Pharmacy Residents Participate in various committees, providing guidance and advice on drug therapy Participate in quality improvement activities, reviewing current practice, identifying problems and recommending and evaluating modifications Maintain workload records and metrics as required Participate in programmes such as Patient Safety and Drug Reaction Reporting by collecting and compiling relevant data as applicable Share responsibility for the orientation of new personnel and act as a role model Maintain a working knowledge of PharmaCare policies and procedures Perform other related duties as assigned Qualifications Current registration as a pharmacist with the College of Pharmacists of British Columbia Two (2) years’ recent, relevant patient care experience or equivalent combination of education, training and experience Valid BC Driver’s Licence and access to a personal vehicle for business‑related purposes Broad knowledge of organisational policies, procedures and standards of care Knowledge of evidence‑based clinical pharmacy practice and medication use evaluation Knowledge of research processes and methodology, and drug distribution processes in a health system setting Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision Effective verbal and written communication skills Ability to establish and maintain effective working relationships with other health practitioners, clients, family members and the public Ability to organise and prioritise work and operate related equipment Physical ability to carry out the duties of the position Important: Recruitment scam warning Please be on alert for recruitment scams.
