Nurse Field Case Manager (FCM) – Ontario Location: 1685 Tech Ave, Mississauga, Ontario, L4W 0A7 Shoppers Drug Mart is innovating health and wellness in Canada. Why this role is important The Nurse Field Case Manager collaborates on ongoing assessment, planning, facilitation, service coordination, evaluation, and advocacy to meet the needs of patients, physicians, and clients. The role delivers quality, cost‑effective treatment outcomes, supports physicians in securing drug coverage, and ensures seamless care delivery both in person and remotely. What you will do Assist healthcare practitioners in completing program enrolment forms and educate them on enrolment requirements. Gather required patient information, including physician signatures and product details. Provide reimbursement assistance to patients and physicians, investigate case‑by‑case reimbursement options, and prepare supporting documentation. Determine patient eligibility and channels for reimbursement, and assist with payer documentation and Special Authorization (SA) submissions. Support physicians with appeal letters, annual prescription renewal, and SA renewal processes. Identify obstacles to coverage, take appropriate action, and find alternative reimbursement channels. Maintain patient files in the program database and educate HCPs and patients about program services. Engage with manufacturers to provide field feedback on services, training, and quality improvement. Collaborate with sales reps and associates and attend scheduled educational events. Report adverse events and maintain high professional standards, ethics, and continuous learning. Assume responsibility for personal and professional development and other duties as required by SDM Specialty Health Network. What you will need Registered Nurse with a current, unrestricted license in Ontario. BScN preferred; a combination of experience and education will be considered. Proficient in reimbursement, Special Authorization, and Appeal letter processes. Recent experience with Patient Support Programs (PSP) is an asset. Strong customer/account focus and consistent delivery of the PSP value proposition. Fluency in both written and spoken French and English is an asset. Five or more years of direct clinical experience. Excellent communication, telephony, leadership, and case‑management skills. Ability to adapt to a changing environment and demonstrate professionalism and ethics. Proficient computer skills, including Microsoft 365. Valid driver’s license and reliable vehicle.
