Location: Various

Reporting to the Program Manager, the incumbent will assist with ongoing development, implementation, and review of individual programs for participants involved with Community Re-entry programs. The programs are holistic in nature and include cognitive, social, communication, and leisure elements, as well as activities of daily living and household management.


ACCOUNTABILITIES INCLUDE BUT ARE NOT LIMITED TO:
  1. Promoting the rights of persons with an acquired brain injury by facilitating independence and empowerment, enhancing quality of life, and educating the community as required.
  2. Provide supportive counselling by promoting participation in social functions, arranging transportation, identifying inappropriate behaviour, and redirecting the participant to alternative action.
  3. Responsible for maintaining a high level of job knowledge, skill, work habits, and professionalism by attending mandatory training, refreshers, participating in education seminars, conferences, and workshops.
    1. Minimum 75% completion of a diploma or 50% completion of a degree in social work, social sciences, psychology, or related field.
    2. Preference for one year of full-time equivalency experience in social services, including experience with implementing behavioural and individual support plans.
    3. Demonstrated ability to work with behavioural, emotional, and mental health issues.
    4. Demonstrated proficiency in Microsoft Office computer applications.
    5. Driver’s license and daily access to a reliable passenger vehicle.
    6. Current certification in CPR and First Aid.
    7. A preferred asset also includes proficiency in speaking/reading French.

      This job requires the incumbent to support participants as needed with physio-therapy routines, ADLs, personal hygiene, lifts, and transfers, as well as completing household duties. When working overnight shifts, the incumbent is required to remain awake and alert through the overnight hours, completing household duties, and assisting participants as needed.
