Job Summary and Requirements This posting is for one full-time (75 hours biweekly) term position until September 15, , and one part-time (60 hours biweekly) term position until December 17, . This job is based in the thriving and diverse city of Whitehorse in the beautiful Yukon with travel to Yukon communities. About us The Yukon Home CareProgramprovides client‑focused health services in the home, improving quality of life and supporting Yukoners to live independently. Home Care offers a range of services tailored to meet individual needs, including support for acute care, chronic and palliative conditions. These services are determined through standardized assessments to assist in creating individualized plans of care; and are supported by an interdisciplinary team, consisting of Physiotherapists, Occupational Therapists, Registered Nurses, Social Workers, Recreational Therapists, and Home Support Workers. Your role Community Care Occupational Therapists provide services to Home Care clients through the Yukon Home Care Program. Continuing Care Occupational Therapists work collaboratively within an interdisciplinary team to meet resident / client needs and maintain quality of life. We are looking for a dynamic individual to provide clinical care services for residents / clients with physical, cognitive, social and / or emotional impairments. The Occupational Therapist is responsible for providing direct care and consultations to enhance, maintain, improve, or restore the ability to participate in meaningful self‑care, leisure, and productive activities. The Occupational Therapist plans, directs, delivers, and evaluates services through direct assessment, interventions, consultations, and assignment of task components to Rehabilitation Assistants. This position also involves providing training to Continuing Care staff.
