NHS Continuing Healthcare (CHC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person's physical and mental health needs arising from disability, accident or illness. It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase.NHS Frimley ICB's East Berkshire Continuing Healthcare Service is seeking to recruit two highly motivated, very well organised and experienced registered nurses with qualifications in LD, MH or General nursing or an Allied professional i.e. Physiotherapist, Occupational Therapist, Social Worker to join the team of CHC assessors to support in the processing of Continuing Healthcare and Funded Nursing Care assessments, reviews and case management.A current driving licence and vehicle used for business use will be required.For an informal discussion, please email in the first instance to a.moorshead@nhs.net or juliet.hammond10@nhs.netAt our Organisation, our selection process ensures we recruit candidates with the right skills and values. You must declare any AI use when submitting your application.Main duties of the jobYou will have knowledge of the National Framework for Continuing Healthcare (Oct 2018) and be able to:Coordinate multi-disciplinary meetings with a range of professionals i.e. social care, care homes, other providers etc.Undertake high quality holistic assessments of patients, clearly identifying needs using assessment tools as required.Face to face reviews/assessments will be required.Manage a caseload of complex cases.Cover as Duty Nurse within the Continuing Healthcare Service.You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines. The postholder will implement the National Framework for Continuing Healthcare and NHS Funded Nursing Care.To complete high quality holistic assessments, reviews and re-assessments in a variety of settings i.e., nursing home and community to enable application of the NHS National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. Face to face reviews/assessments will be required.Work as part of a multi-disciplinary team, acting as a resource to provide nursing advice in relation to appropriate placement and management of clients.Provide accurate advice and information to all regarding recommendations following completion of assessment process.To manage a generic caseload of clients following their placements for Continuing Healthcare and function as a Duty Nurse within the Continuing Healthcare Team. These duties will require the postholder to work with other organisations.Support teams to ensure that the portfolio of tasks/projects is planned, managed and delivered effectively.Support and inform the targeting of resources, monitoring, implementation and evaluation of the tasks/projects by providing high quality support including complex information and analysis, communications and stakeholder management.Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialities, functions and projects.Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.The job description and person specification are an outline of the tasks, responsibilities and outcomes required of the role. The job holder will carry out any other duties as may reasonably be required by their line manager.The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the Department and the Organisation.Person SpecificationKnowledge, Training and ExperienceEducated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area.Further training or significant experience in project management, financial management or supporting change management processes.Experience and understanding of evaluating and measuring the performance of health services.Experience in communications and stakeholder management.At least 3 years post registration experience of working within a multi-disciplinary team.Experience of undertaking highly complex, clinical assessments.Experience of working in older persons care in NHS or Independent setting.Experience of working with patients with multiple or complex care needs.Experience of assessing complex needs and planning and evaluating complex care packages.A good understanding of the health and social care environment and roles and responsibilities within it.Knowledge and understanding of safeguarding of vulnerable adults.Knowledge of Mental Capacity Act.Knowledge and understanding of quality, clinical audit and evidence of practice.Liaison with outside agencies.Teaching experience.Previous experience in similar role in public sector.Audit data analysis- ability to analyse data and use it for audit purposes.In depth knowledge of NHS Continuing Healthcare legislation.Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.Skills for communication on complex matters and difficult situations, requiring persuasion and influence.Skills for nurturing key relationships and collaborative working.AnalyticalAbility to analyse and interpret information, pre-empt and evaluate, to produce concise and accurate written reports.Problem solving skills and ability to respond to sudden unexpected demands.Strategic thinking - ability to anticipate.Attention to detail combined with the ability to extract key information from complex analysis.Independent thinker with demonstrated good judgement, problem-solving and analytical skills.Takes decisions on difficult and contentious issues where they may be a number of courses of action.Planning SkillsAbility to organise and prioritise work (including delegation to others) and to complete work within set time scales.Management SkillsAbility to build trust and confidence and demonstrate integrity in the provision of advice and support.Skills for managing workload ensuring they meet NHSE targets.OtherAn ability to maintain confidentiality and trust.Used to working in a busy environment.Adaptability, flexibility and ability to cope with uncertainty and change.Commitment to continuing professional development.Professional calm and efficient manner.Demonstrate a strong desire to improve performance and make a difference by focusing on goals.Car driver - ability to travel independently.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
