Primary Care Nurse

Primary Care Nurse (RN) - Urgent and Primary Care Centre (UPCC) Registered Nurse (RN) position at Vancouver Coastal Health (VCH) within the City Centre Urgent and Primary Care Centre team in Vancouver, BC. Salary range: CAD $41.42 to $55.91 per hour. Responsibilities Triage, assess, plan, implement, and evaluate nursing care for patients and families who require high priority care within 24 hours. Develop and maintain partnerships with primary care providers in accordance with the local Community of Care (COC) service delivery model and primary care network. Use approved triage standards to apply critical thinking skills within safe limits of patient care. Apply chronic disease management, palliative, health management, and/or self-care management principles to conduct assessments, identify problems, provide direct care, and address issues or variances from the plan of care. Act as a resource for the interdisciplinary team by providing information based on nursing theory and practice related to patients with primary level care needs. Education & Experience Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Two (2) years’ recent clinical experience that is relevant to the designated clinical area, or an equivalent combination of education, training, and experience. Knowledge, Skills & Abilities Demonstrated ability in applying critical thinking skills within safe limits of patient care. Knowledge and skills within primary care and patient‑and‑family‑centred models of care. Knowledge of relevant Professional Regulations and Standards (e.g., Community Health Nurse of Canada competencies). Evidence‑based clinical practice related to primary care. Knowledge of provincial acts, regulations, and program policies related to home, community, and primary care. Knowledge of acute, chronic disease, mental health, substance use, palliative, and self‑management support. Culturally safe care to diverse populations. Trauma‑Informed Practice, Harm Reduction, and Recovery‑Oriented Care. Effective communication and conflict resolution. Independent work and priority setting. Understanding of other health disciplines and their roles in patient care. Use of research in practice and adult education principles for teaching patients and families. CPR skills and physical ability to perform duties. Operation of related equipment. The hours of work, including days off and work area, may be subject to change consistent with operational requirements and the applicable Collective Agreement. As per Ministry of Health policy, all health care workers working in publicly‑funded facilities are required to report receipt of certain vaccines or history of infections.
