Job Title: (899) Case Manager, Youth and FamilyEmployer: Bluebonnet Trails Community ServicesLocation: SeguinSalary: $21.46 – $30.71/hrJob Type: Full-timeOverviewThe person in this position will work with youth and their families to deliver case management, skills training and medication education primarily in community-based settings, such as homes and schools. This position uses evidenced based treatment, individualized levels of care, and person-centered recovery planning to best serve youth and their families. The person in this position will assist youth and their families with accessing community resources and provide rehabilitative services and crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team.Essential FunctionsBuilds and maintains a therapeutic relationship with individuals through engagement.Provides psychosocial rehabilitation along with linking individuals with community support.Performs assessments with individuals identifying their strengths and needs.Completes Person Centered Recovery Plans developing goals and objectives with individuals.Completes documentation in a timely manner.The RoleHas initiative and works well in ambiguityEngages individuals based on personalized Level of CareCompletes Assessments within expected time frame (180 days)Completes Recovery Plans to guide treatment; is a member of an interdisciplinary treatment teamHas access to ongoing TrainingQualificationsMinimum Qualifications: A Bachelor’s degree from an accredited college or university with a major or equivalent coursework in a social, behavioral, or human service field and knowledge of mental illness. Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment. If the degree is not in a human services field, a minimum of 24 college credits in a human services field are required.Preferred Qualifications: One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred.Salary DetailsSalary range may reflect differentials: Up to $4.00/hour with BTCS tenure; $1.00/hr Master’s degree; $0.75/hr Bilingual; Up to $2.00/hour for prior community center experience in a similar position; $1.00/hour advanced licensure; $0.50/hour LCDC. $200 Monthly Service Delivery Bonus.OtherClosing Date: Open until filled. #J-18808-Ljbffr
