LOCATION: Espanola or Sudbury (Positionwill service both locations; Hybrid model of workapplies to perform in-person patient visits, work  remotelyand from the office when required)

EFFECTIVEDATE: Immediately to April 30, 2025

POSITION SUMMARY:

TheRegistered Nurse - Rapid Response focuses on the care during transition fromacute care to home care for eligible complex/chronic adults and medicallyfragile children.

NATURE & SCOPE:

· Provides the first in-home nursing visit within twenty-four (24)hours of hospital discharge for patients with complex/chronic diseasesincluding CHF, COPD, Diabetes or other;

· Completes a clinical, problem-based head to toe assessment;

· Confirms the patient’s hospital discharge plan;

· Performs medication reconciliation for patient and reviewsmedication protocol with patient and caregiver;

· Utilizes a "teach back" approach to help the patient andfamily understand the care plan, treatments, symptom management, and when/whoto ask for help;

· Initiates communication with the patient's primary care provider,and ensures follow up with primary care within seven (7) days;

· Refers to longer-term community programs through the CareCoordinator as required, once goals of Rapid Response program are met.

QUALIFICATIONS:

· Registered Nurse in good standing withthe College of Nurses of Ontario;

· Minimum 3-5 years clinical experienceworking as a Registered Nurse;

· Recent and relevant experienceproviding direct (hands-on) nursing care within the areas of Paediatrics,Chronic Disease Management, Medicine or Cardiology.Preference will be given to experience withinthe last 2-3 years;

· Certificate in Geriatric Nursing (GNC)or Certified Nurse in Critical Care Pediatrics (CNCCP) an asset;

· Superior clinical assessment skills;

· Ability to provide quality clinicalservice delivery in conjunction with patients and other stakeholders;

· Ability to work independently;

· Strong critical thinking and problemsolving skills;

· Effective interpersonal andcommunications skills;

· Ability to provide health coaching anddisease-specific education to patients;

· Advanced oral and written proficiencyin English is essential;

  • Must have valid driver’s licenseand access to a vehicle, as regular travel to patient homes in Espanolaand Sudbury will be required;
