Palliser Youth Navigator
We Never Give up.
Wood's Homes is currently seeking one, temporary full-time Youth & Family Counsellor I or II to join us for a pilot project in collaboration with the Palliser School Division as a Youth Systems Navigator. This is a term position until December of 2024.
The Youth Systems Navigator provides direct and consistent one-to-one navigation with youth 12 to 19 within the Palliser School Division. One-to-one navigation consists of regular face-to-face and virtual meetings focused on the identification and support of immediate barriers related to all youth-related risk factors, including overall mental health, well-being, peer, and family dynamics. Youth may also need support with activities of daily living, and System Navigators can engage them in collaborative problem-solving and motivational interviewing to meet their identified needs. Youth Systems Navigators can also serve as youth population health experts who are aware of current community trends and can provide both direct support to clients, as well as serve in consultative and collaborative support capacities to better assist other third parties in developing their own case plans and supporting services with identified youth.
Responsibilities:
- Collaboration with Palliser School Division and all other Alberta Mental Health Grant funded service providers.
- Operate as a tier 3 and 4 Youth Systems Navigator based on current Palliser School Division criteria.
- Support systems navigation, case management and service delivery with youth, their families and/or caregivers where appropriate.
- Support both identified and self-identified youth and families through wraparound case management, case consultation, systems meetings, rapport building, and participation at community appointments when and where required.
- Targeted outreach to identified students, families, and Palliser School Division schools to ensure that relevant information and services are easily accessible to youth in need.
- Participate in supervision, team meetings, retreats, planning sessions, and in-service training.
- Participate in outreach activities at schools, in the community, and other-related youth events.
- Must be an effective, mature, and proper role model for youth.
- Knowledge of Indigenous cultural practices, traditional ways of knowing, and helping to build appropriate cultural resources into daily wellness activities for young people and families (would be an asset).
- Participate in research of trends in our community and best practices on how to mitigate those trends and present these findings to Palliser School Division.
- Actively build and support relationships and rapport with other youth serving agencies in the Palliser School Division catchment area.
- Maintain proper administrative and client records.
- Complete all required health and safety training.
- Regular use of Aware 360 safety app while interacting with clients in the community.
- Report workplace hazards, unsafe or unhealthy conditions, or incidents (including near misses, injuries, or equipment damage) to their direct supervisor.
- Show initiative, enthusiasm, and creativity in all aspects of day-to-day duties (e.g., administrative, clinical skill development, and interactions with others).
- Execute other responsibilities as assigned by program leadership.
- Diploma or Undergraduate Degree in the Social Sciences, including Child and Youth Care, Social Work, Addictions, or Psychology.
- Minimum of two (2) to four (4) years of related experience in working with young people in a community or mental health setting.
- Current CPR and First Aid Certification.
- Training in trauma-informed care and interventions associated with children’s mental health.
- Awareness and ability to help access community resources and support for youth and their families.
- Personal suitability is an important consideration.
- Indigenous knowledge and life experience would be an asset.
- Current proof of professional registration may be needed for certain diplomas or degrees.
- Must have a full, non-GDL class 5 driver’s license and own transportation.
