What will you do?

· In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans

· Linkpatients with service providers

· Coordinate and monitor care plan delivery

· Establish a helping relationship with patients and their families

· Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected

What must youhave?

  • Membership, in good standing, with theapplicable regulatory body:

o College of Nurses of Ontario

o College of Physiotherapists of Ontario

o College of Occupational Therapists of Ontario

o Ontario College of Social Workers and SocialService Workers

  • Recent experiencein community health or a related field
  • Knowledge ofthe health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving, anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
  • Establishedability to accurately complete required documentation, reports and forms
  • A validdriver’s licence and access to a reliable vehicle
  • We have a mandatory COVID-19vaccination policy. As a condition of employment, all employees arerequired to submit proof of COVID-19 vaccination status prior to startdate.

What wouldgive you the edge?

· Experienceworking with diverse patient groups, e.g., multicultural, homeless, palliative,acquired brain injury (ABI), mental health, geriatrics, pediatrics
