Role & Responsibilities:

  • Act as a point of contact for clinical care matters within Anglican Care’s Residential Aged Care Home.
  • Provide Individual Case Management support to the Residential Care Manager to ensure a high standard of care is provided to consumers.
  • Organise and facilitate case conferencing to partner with consumers and their representatives in the development of care provisions and plans.
  • Assist in meeting the goals and objectives of Anglican Care.
  • Ensure the highest standard of consumer care through audit processes.
  • Develop and implement all policies related to the provision of Resident Care Services.
  • Provide guidance and leadership to other staff to ensure a high standard of care is provided to consumers.
