Description

Umoja Community Mosaic was founded as Soccer Without Boundaries was in 2010 as a drop-in soccer club to help immigrant and low-income children get involved in their community. Since then, we have grown in a community-owned way and introduced programs like the culturally appropriate food pantry and delivery, after-school programs, tutoring, leadership development, anti-racism, and inclusion programming.
We are currently looking for a new team member to join us as our full time Community Support Worker (CSW).
Reporting to the Community Program Lead, the Community Support Worker works directly with youth program participants and their families, providing wrap-around supports to support community belonging and pro-social outcomes. The CSW will provide person-centred and strength-based care in the context of Asset-Based Community Development initiatives, including systems navigation and referrals, and tracking progress.
As our Community Support Worker, your responsibilities will include:

Participant Engagement

  • Developing relationships of trust and reciprocity with program participants and their families
  • Providing uninterrupted, coordinated care for participants across different Umoja programs
  • Provide culturally appropriate services including communications, community participation, conflict resolution, and natural support development.

Case Management

  • Engage youth participants in strength-based goal-setting
  • Provide ongoing supports to participants as they pursue their goals, including:
    • System Navigation and Referrals
    • Facilitating access to trusted adults
    • Facilitating access to natural supports and community resources
    • Coaching, counselling, and life skill development
  • Engage youth participants in continuous assessment of own goals, facilitating adaptation and celebrating success
  • Collect stories, quotes and recommendations from participants and their families
  • Participate in data collection, entry, interpretation, and reporting as required
  • Provide specialized advice and guidance to Umoja staff
  • Support organization-wide activities as required

The right candidate will demonstrate on their resume:

  • 2+ years of support experience in local or international setting;
  • Proven ability to engage community members in a strength-based modality;
  • Ability to build and maintain effective, mutually supportive relationships with team members across diverse identity factors, ethno-cultural identities and languages.

  • Strong written and verbal communication skills
