Detailed Overview

Reporting to the Program Coordinator, Early Psychosis Intervention (EPI) Program, functions as a member of a community based multi-disciplinary team. Provide direct and indirect service to adolescents and young adults who meet the criteria for the Early Psychosis Intervention (EPI) Program. Conducts comprehensive psycho-social assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices and educates and provides consultation to patient, families and healthcare professionals. Develops, coordinates and delivers in-services, presentations, psycho-educational and therapy groups and carries out research activities in collaboration with the multi-disciplinary team.

This position may be required to provide services at Fraser Health Community South Fraser and Fraser Health Community Simon Fraser.


Responsibilities

  1. Provides single entry intake of referrals into the EPI Program from hospitals, mental health centres, and the community. Provides comprehensive mental health assessments, outreach, and referrals for treatment by methods such as conducting mental status exams and assessing for psychosis in less intrusive settings, such as the school or home when necessary, facilitating psychiatric assessments as needed, and providing support and education to families during the assessment process.
  2. Participates in ongoing development of the group therapy component of the program by co-facilitating adolescent, adult, and family groups, using a variety of group therapy approaches.
  3. Provides outreach and monitoring of those clients identified as being at risk for psychosis, (at risk mental state), by methods such as providing psycho-education, assessing mental state, and maintaining contact with these clients and their families in community settings or through the telephone as needed.
  4. Develops, coordinates and delivers in-services, presentations, psycho-educational and therapy groups and/or training related to the EPI Program by methods such as collaborating with members of the multi-disciplinary team, determining relevant content and required audiovisual/handout aids and conducting literature searches.
  5. Provides consultative and direct services to clients and families by providing information for clients and families regarding community resources and access to these resources in order to facilitate an effective continuum of services as per the guidelines of the EPI Program.
  6. Informs regional staff, community groups, schools, and agencies of the role and function of the EPI Program by providing information informally and formally during in-services. Participates as a member of local committees and planning bodies by methods such as engaging in program/practice evaluation, identifying opportunities for change, assessing available resources, initiating solutions, analyzing outcomes and identifying areas for improved efficiency and effectiveness in service delivery to enhance patient care for services and reduce service gaps in the community.
  7. Maintains clinical records by recording information such as intakes, assessments, treatment formulations, care plans, and progress notes. Maintains statistical information on client activity in accordance with established policies, standards, and procedures, including clinical record standards of Mental Health Services. Obtains consent forms for release of client information.
  8. Carries out research activities and program planning and development relevant to the needs of the patient, families and community in collaboration with the multi-disciplinary team by methods such as collecting research data and identifying current trends/gaps in literature.
  9. Completes statistics, case documentation, standardized assessment reports, discharge summaries and other relevant documentation according to the policies and procedures established by the program, and for the purposes of outcome research.
  10. Participates in quality assurance activities by performing duties such as gathering consumer related information and statistics, developing and evaluating clinical protocols, and identifying service needs. Provides input to the Program Coordinator regarding policy issues, service gaps, and program development, maintenance, and evaluation needs.
  11. Attends team meetings, multi-disciplinary meetings and clinical consultation as required.
  12. Performs other related duties as required.

Qualifications

Education and Experience

Master's Degree in Social Work, Clinical Counselling, or other relevant health related discipline.

One (1) year recent, related experience in Adolescent Mental Health, including assessment, family intervention and group therapy or an equivalent combination of education, training, and experience. Eligible for registration with relevant professional association.Valid B.C. Driver's license, and have access to own vehicle, with appropriate business insurance coverage.



Skills and Abilities

  • Comprehensive knowledge and skill in working with adolescents with mental health problems, family therapy, and group therapy.
  • Comprehensive knowledge of community response procedures, mental health assessment, general counselling principles and intervention techniques, abnormal psychology, and DSM IV multiaxial diagnosis.
  • Demonstrated knowledge and understanding of the diagnostic criteria pertaining to psychiatric diagnoses, and particularly early psychosis.
  • Demonstrated knowledge of family dynamics and family systems theory, crisis intervention, child protection and related legislation, and the full continuum of services available to youth and families.
  • Demonstrated knowledge of the development stages of an individual within the family life cycle and the related psychosocial issues that may occur.
  • Demonstrated skill in conducting comprehensive mental health assessments.
  • Demonstrated knowledge and experience in the field of alcohol and drug misuse.
  • Demonstrated knowledge and skill in the development of and facilitating psycho-educational groups for a broad range of clients.
  • Demonstrated knowledge and understanding of pharmacological issues related to the identified population being served by this program.
  • Demonstrated knowledge and understanding of the unique aspects of providing service across the hospital/community interface.
  • Demonstrated ability to work independently and collaboratively utilizing a multi-disciplinary team model.
  • Demonstrated ability to organize work, and to set and adjust priorities in relation to identified risk factors.
  • Demonstrated ability to participate in clinical research and program evaluation activities relevant to the needs of clients, families, and the community.
  • Demonstrated ability to communicate effectively and diplomatically, both verbally and in writing.
  • Demonstrated ability to proactively and effectively resolve issues impacting on quality and continuity of care.
  • Demonstrated ability to work in high volume/high stress environment with strong conflict resolution and problem solving skills.
  • Demonstrated ability to operate related equipment, including computer software.
