Practice Information Practice Name: Vital Path Addiction Clinic Practice Setting: Community Health Centre (CHC) Other Providers: Community PCN: 3: Northeast EMR: MOIS About the Clinic The Vital Path Addictions Clinic is a busy, well‑managed clinic whose goal is to provide wrap‑around medical support for people in treatment and post‑treatment. The clinic currently has three addiction physicians on staff and is looking to grow its team. Join their dynamic, collaborative environment and enjoy the opportunity to work with a dedicated team of professionals. The Clinic Offers 1 exam room Open offices on Tuesdays, Thursdays and Fridays. Our clinic specializes in addiction treatment, mental health, and recovery, as well as other related services. The patients are in the treatment center, sober living or alumni of the treatment program. We have over 400 beds in population – not everyone needs to see the doctor, but we need someone on site to help with the general needs of those who have exited the cycle of addiction to a better life. Allied Health: We have 3 MOAs, 2 NP, 1 RN and 1 psychotherapist. We also have access to elders. Parking/Bike/Transit information: Easily accessible by transit and parking. Hours: The clinic is subject to doctors’ hours. We prefer 10:00 a.m. – 4:00 p.m. Qualifications Fully or provisionally licensed. Experience in addiction medicine or related areas preferred. Responsibilities Provide medical care to patients in treatment and post‑treatment phases. Support general health needs of patients who have exited the cycle of addiction.
