Under the direct supervision of the Chief Social Worker, assists in performing mid-level rehabilitative planning and instructional work for patients with mental impairments. Additionally, assists with the development of a comprehensive, structured, hospital-wide recreational activity program with some supervisory responsibilities. Work involves assisting with identifying clients' recreation and rehabilitative needs and providing the necessary instruction outlined in conjunction with the Registered Therapist in teaching self-help or self-care to assist clients in attaining their optimum levels of independent living. Provides services in groups or on an individual basis. Duties also include providing activity services at Bonham Day Program as well as on the units, as needed. Supervises the work of others. Performs assigned duties alone or as part of a multidisciplinary team. Works under moderate supervision, with moderate latitude for the use of initiative and independent judgment. Employee must understand that duties, location of work and shift patterns are subject to change as to best meet the needs of the department, program, or hospital.
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
This position may also be eligible to earn additional pay for work performed on maximum security locations. Essential Job Functions:
Attends work on a regular basis and may be asked to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Independently leads and assists in the development of activity-based functional and recreational programs and activities using relevant tasks appropriate for client population. Collaborates with Registered Therapist to teach skills necessary for clients' achievement of independent living, teaches skills relevant to vocational activities. Instructs clients in functional activities to include, but not limited to, meal planning and prep, public transportation, ADL and IADL activities, clothing management. Schedules and instructs clients in therapeutic/wellness activities to include, but not limited to, exercise, arts and crafts, outdoor sports. Selects appropriate activities based on patient function, abilities, and interests. Collaborates with Registered Therapist to provides treatment team with appropriate feedback on patient behavior and response to activities in order to modify or enhance treatment plan goals and targets. Supervises rehab tech activities on unit and off. Services are provided individually and in groups. Position provides activity services at the Bonham Day Program.
Effectively manages a wide variety of disabilities, cognitive levels, and behavioral problems in the therapeutic setting. Establishes and maintains a relaxed atmosphere of cooperation among patients and staff while in the therapeutic setting. Understands the differences in approach or interaction for various psychiatric diagnoses, intellectual disabilities, and geriatric populations. Assists Rehab Director in program development to include development of teaching material, lesson plans, assist in development of patient independent living skills. Assists the Rehab Director in development and implementing of individual plans based on comprehensive overview of the program. 4x/year, minimally, the program is reviewed for effectiveness through patient feedback. Administers vocational evaluations.
Responsibly prepares supplies / equipment for all scheduled activities. Prepares materials for therapeutic activities and maintains an adequate supply of materials.
Completes all documentation in computer-based medical record as required in a timely and relevant manner. Documents accordingly on observations as appropriate in reporting the patient’s behaviors, reactions, and progress. Progress notes are completed withing Medicare, facility, and department guidelines. Completes monthly reports of patients served and activities presented.
Attends all mandatory programmatic in-services to maintain compliance with program standards. Webinars, grand rounds, professional association meetings, etc. Complete all required annual training and maintains. 10 hours of continuing education annually to maintain skill/knowledge for effective program content.
Demonstrate and mentor safety as a personal responsibility and comply with all established safety rules, regulations, and procedures to include the procurement of applicable SDS sheets, monitoring sharps, disinfecting equipment, etc.
Performs other duties as assigned. Knowledge of the rehabilitative process, and of skills and adaptations necessary for employment placement of persons with disabilities.
To plan an individualized program of instruction with, or on behalf, of the client; to communicate effectively; and to supervise the work of others.
Demonstrates skill and capability of accurately observing and identifying significant patient behaviors and possesses the capability of effectively communicating and documenting in progress notes.
Ability/willingness to work with supervisor, co-workers, and follow established policy and procedures.
Knowledge of therapeutic methods and techniques in the rehabilitative therapy area of specialization of mental illness and mental disabilities in a safe manner.
Ability to maintain inventory of supplies needed for activities/services.
Ability to possess a wide variety of leisure skills and a firm knowledge of the value and role of Recreation/Leisure activities. Strong working knowledge of group dynamics and application to Rehabilitation Therapy Services.
Ability to continually present self as a role model to all persons in contact with.
Ability to demonstrate effective skills and techniques in motivating patient participation.
Ability to describe and demonstrate importance of positive words, creativity, consistency, genuineness, enthusiasm, and different roles in relating to patients.
Knowledge of Rad Plus, ability to retrieve and print documents.
