**Job Description:
Key Responsibilities- Assess patient needs and develop individualized care plans to promote optimal health outcomes.
- Provide high-quality nursing care to patients in their homes, focusing on prevention, education, and support.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.
- Stay up-to-date with industry developments and best practices to maintain expertise and provide evidence-based care.
- Bachelor of Science in Nursing (BScN) or equivalent.
- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS).
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Strong problem-solving and critical thinking skills, with a focus on patient-centered care.
