Job Summary

We are seeking a skilled Human Services Counselor to join our team at Vitalité Health Network. As a key member of our interdisciplinary team, you will play a vital role in supporting clients with mental health needs.

Key Responsibilities
  • Evaluate and support clients in developing and maintaining their autonomy, in accordance with the interdisciplinary intervention plan.
  • Implement treatment and rehabilitation programs to promote continuity of care for assigned clients.
  • Collaborate with the clinical team to provide comprehensive care and support services.
  • Organize work to promote continuity of care for clients, guided by clinical principles of rehabilitation, risk, needs, and responsiveness.
Requirements
  • High school diploma plus an additional program or course of over six months and up to one year in community integration or the equivalent.
  • Current Basic Life Support (BLS) certification.
  • Advanced non-violent crisis intervention training (CPI) or a commitment to take this training within a set schedule.
  • Prevention and Management of Suicidal Behaviour training or a commitment to take this training within a set schedule.
  • Good judgment and organizational skills.
  • Ability to work independently and as part of a team.
  • Physical ability to perform the assigned work.
  • Good work history (performance and attendance).
  • Adherence to professional ethical principles, the Network's management philosophy, and organizational values.
  • Compliance with the rules of confidentiality set by Vitalité Health Network.
