The Toronto Grace Health Centre (TGHC) is a specialized care facility that provides a range of services to individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. Our goal is to facilitate patient flow through the healthcare system and support recovery, enabling individuals to return home or transition to Long Term Care facilities in their communities.

Registered Practical Nurse (RPN) Opportunity
We are currently seeking a Registered Practical Nurse to join our Specialized Care Centre team at our Scarborough location. This is a unique opportunity to work in a dynamic and supportive environment, providing high-quality care to patients with complex needs.
Key Responsibilities:
- Provide excellent patient and family-centered nursing care in a complex patient environment
- Establish caring and therapeutic relationships with patients and their families
- Promote health and/or treat health conditions through supportive, preventative, therapeutic, palliative, and rehabilitative means
Qualifications:
- Current Certificate of Competence issued by the College of Nurses' of Ontario
- Current Basic Cardiac Life Support (BCLS) Certification
- Minimum 1 year of nursing experience, with experience in Continuing Complex Care or Long Term Care an asset
- Ability to work as a member of the chronic care team within the unit's mission statement
- Experience working with medically complex patients in acute care hospital an asset
- Experience with mechanical ventilation an asset
- Able to establish therapeutic relationships with patients and families
- Commitment to continuous improvement, learning, and evidence-based practice
- Demonstrated resilience to adapt in an ever-changing environment
- Demonstrated problem-solving and critical thinking skills
- Highly motivated, compassionate individual with excellent interpersonal and communication skills
- Excellent written and verbal skills in English
- Strong time management skills, excellent attendance record
- Demonstrated skills in applicable computer software applications
- Ability to meet bone fide occupational requirements as outlined in the Job Demands Analysis
