Overview

We are looking for a Community Support Worker (CSW) to work in our Transitional Care Unit. The work is within a safe community-based transitional environment where patients will be supported in achieving or maintaining optimum functioning while awaiting their next care destination either private retirement home, or long-term care. The program's aim is to provide short-term transitional care with an estimated length of stay of 45 days. We are hiring for a 4-month contract.

Full Time Shift work within a 12-hour model - 7am – 7pm.

What The Role Involves

  • Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions
  • Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety
  • Providing cognitive, social and/or emotional support to the patient
  • Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices.

  • Availability to work non-traditional hours, such as early morning, evenings, and alternate weekends
  • A valid driver's license and vehicle
  • Valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program
  • To protect our patients, we require current vulnerable sector check screening, a clear background check and proof of COVID 19 vaccination*
