Job Summary

We are seeking a highly skilled and experienced Pharmacist to join our team at the University Health Network. As a key member of our Outpatient Pharmacy team, you will play a critical role in ensuring the safe and effective dispensing of medications to our patients.

Key Responsibilities
  • Medication Management: Review, verify, and dispense prescribed medication orders, ensuring accuracy and appropriateness.
  • Drug Therapy Monitoring: Identify, resolve, and prevent drug-related problems, and monitor medication therapy for effectiveness, toxicity, and adherence to evidence-based medicine guidelines.
  • Medication Reconciliation: Conduct medication reconciliation to ensure accurate and up-to-date medication lists for patients.
  • Incident Response: Respond to medication incidents and promote safe medication practices on the unit.
  • Drug Information: Respond to drug information questions and provide expert advice to healthcare professionals.
  • Supervision: Provide direct and/or functional supervision of technician staff, ensuring they are equipped to perform their duties effectively.
Requirements
  • Education: Completion of a university degree in Pharmacy (BSc or PharmD) or equivalent required.
  • Certifications: Certificate of competency from the Pharmacy Examination Board of Canada and active registration with the Ontario College of Pharmacists (Part A) required.
  • Experience: Familiarity with common drug regimens and dispensing processes for specialty medications, as well as experience working as part of a multi-disciplinary team.
  • Skills: Strong interpersonal, communication, and customer service skills, with excellent verbal and written communication skills.
  • Knowledge: Working knowledge of adherence programs, including compliance packaging, refill synchronization, and refill management, as well as manual billing and out-of-province billing processes.
