We are seeking a highly skilled and compassionate Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health Authority. As a key member of our interprofessional care team, you will provide assessment, treatment, consultation, and follow-up services to clients and their caregivers.
Key Responsibilities- Client Assessment and Care Planning: Conduct comprehensive nursing assessments, develop and implement individualized care plans, and provide education to clients and their caregivers on mental health and substance use issues.
- Collaboration and Consultation: Work collaboratively with the interprofessional care team, including MH&SUS Physicians, Primary Care Providers, and other healthcare professionals, to identify and resolve issues and adjust care plans as needed.
- Community Liaison and Referral: Facilitate clients' utilization of appropriate community services by liaising and collaborating with key service providers to ensure timely and seamless care.
- Documentation and Reporting: Accurately document assessments, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Professional Development: Identify own learning needs/goals, review these with the Team Coordinator, and engage in activities to meet these needs/goals.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent related experience or an equivalent combination of education, training, and experience.
- Valid Class 5 B.C. Driver's License and access to a personal vehicle with appropriate insurance coverage.
