We are seeking a highly skilled and compassionate Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health. As a Community Mental Health Specialist, you will play a vital role in providing treatment and care management services to individuals with mental illness and concurrent disorders.
Key Responsibilities- Client Care Coordination: Collaborate with interprofessional care teams to coordinate care for clients, perform intake functions, conduct mental health assessments, and develop comprehensive treatment plans.
- Nursing Care Planning: Plan and provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and implementing treatment modalities such as ADLs, wound care, and medication administration.
- Therapeutic Relationships: Establish and maintain therapeutic relationships with clients and their informal support networks, engaging stakeholders in treatment planning and evaluation, and promoting self-care and informed decision-making.
- Treatment Services: Provide treatment services, including crisis intervention, individual counseling, and education, using a variety of theoretical models and approaches.
- Case Conferences and Collaboration: Participate in case conferences and meetings with clients, family, service providers, and interprofessional team members to identify client mental health issues, coordinate care, and ensure a coordinated approach to treatment.
- Advocacy and Support: Act as a client advocate, protecting and promoting clients' rights to autonomy, respect, privacy, dignity, and access to information.
- Education and Experience: Graduation from an approved School of Nursing or Psychiatric Nursing, with two years of recent experience providing treatment services to individuals with mental health and/or concurrent disorders.
- Skills and Abilities: Ability to complete initial and ongoing mental health and substance use client assessments, provide nursing care through appropriate technical, therapeutic, and safety interventions, and communicate effectively with clients, families, and healthcare teams.
