About the Role

This is an exciting opportunity to join our team as a Health Care Worker in our Unit Based Programs. As a key member of our team, you will play a vital role in supporting the development and maintenance of skills that are relevant for patients' reintegration in the community setting.

Key Responsibilities
  • Support patients in developing skills relevant for reintegration in the community setting through one-on-one work on the unit or in the community, following the principles of psychosocial rehabilitation and supporting small community outings.
  • Prepare for groups by ensuring program materials are ready, reviewing and preparing group content, and cleaning up the group space as required.
  • Work alongside patients to demonstrate and teach skills such as laundry, making beds, general cleaning, personal hygiene support, accompanying patients in the community, and orienting patients to resources in the community.
  • Establish and maintain therapeutic relationships with patients and families while fostering a culture of patient and staff safety.
  • Work collaboratively with the interdisciplinary team by participating in care planning meetings to identify patient strengths, limitations, and areas for development.
  • Ensure the safety of staff and patients by reviewing patient information and communicating reports and records as necessary.
Requirements
  • Grade 10 plus graduation from a recognized Nursing Assistant Program or Care Attendant Program, or an equivalent combination of education, training, and experience.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to deal with others effectively.
  • Physical ability to carry out the duties of the position.
  • Ability to organize work.
  • Ability to operate related equipment.
  • Commitment to developing knowledge and understanding of legislative obligations and provincial commitments related to Indigenous reconciliation.
