Job Summary The Registered Nurse will be responsible for assessing, planning, providing, and evaluating nursing care to patients. This will involve working collaboratively with the interprofessional care team to provide person-centered care and promoting a culture of respect, caring, and trust. Responsibilities • Assess patients and collect information from a variety of sources to make informed nursing diagnoses and develop effective care plans • Collaborate with patients and their families/care givers to develop and implement care plans that promote patient autonomy and dignity • Provide direct patient care, including administering medications, performing procedures, and providing education and support to patients and families/care givers • Participate in the care and discharge planning process for patients, ensuring that care plans are established, implemented, and evaluated effectively • Coordinate nursing care for patients, setting priorities, assigning and delegating tasks to support staff, and making referrals to the interprofessional care team as needed • Collaborate with members of the interprofessional care team to identify and resolve issues, adjust care plans as required, and respond to changes in patient condition • Teach patients and families/care givers about illness prevention, health restoration, and health maintenance to support patient self-care and discharge planning • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures Qualifications • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) • Completion of the applicable post graduate specialty nursing course/program and/or willingness to complete the applicable post graduate specialty nursing courses/program • Current certification in Basic Cardiac Life Support (BCLS) • Ability to communicate effectively with patients, families/care givers, and members of the interprofessional care team • Ability to promote person-centered care and demonstrate empathy, trust, and respect for patients and families/care givers • Ability to integrate and evaluate data from multiple sources to problem-solve effectively • Ability to teach and instruct patients and families/care givers about topics essential to their health care and well-being • Ability to organize and prioritize work and collaborate with members of the interprofessional care team About Fraser Health Fraser Health is a leading healthcare provider in British Columbia, serving a diverse population of over two million people.
