We have an exciting position available at Fraser Health, where you will work in a collaborative team environment to provide expert pharmaceutical care and education in a designated specialty area. As a Clinical Pharmacy Specialist, you will be responsible for: • Providing consultation services to internal and external health care professionals and applying advanced level of theory to assess, plan, implement and evaluate the delivery of pharmaceutical care in the designated specialty area. • Providing clinical direction on complex cases, including the use of evidence-based approaches to evaluating drug therapy, attending at patient care rounds, reviewing medication histories, providing drug information, patient education and discharge consultations and providing recommendations in the planning and treatment assessment policies and procedures in the designated specialty area. • Ensuring quality assurance programs for the designated specialty area are maintained and policies and procedures followed according to departmental guidelines; identifying and troubleshooting inconsistencies; developing standards, ensuring regular audits are conducted and taking corrective action as required. • Providing academic teaching in the designated specialty area to undergraduate and post-graduate students, including planning, organizing and evaluating completion of hospital-based Pharmacy programs in cooperation with the Faculty of Pharmaceutical Sciences, University of British Columbia. • Identifying and initiating clinical drug research, as appropriate, by developing hypotheses and research processes, overseeing research activities and/or coordinating the involvement of research assistants, directed studies students, residents and pharmacists in research activities. • Conducting pharmacological education/in-service sessions in designated specialty area, as well as orientation sessions for pharmacy staff, program team members and other health care staff. • Participating in the Pharmacy and Therapeutics Committee either through membership and/or preparation of materials for formulary decisions and policy-making decisions by evaluating and making written recommendations regarding formulary requests, conducting relevant research, writing up a monograph or drug assessment for presentation at the P&T Committee, and providing necessary information to all members of the health care team. • Participating in the planning, implementation and evaluation of department therapeutic cost effectiveness and cost restraint programs. • Representing the department on multidisciplinary care committees, and at external events such as conferences, and regional planning groups relative to the designated specialty area. • Providing pharmaceutical care to patients in designated specialty area; reviewing patients' charts and through collaboration with patients and other health care professionals identifies, prevents and solves drug-related problems.
