We are seeking a skilled Primary Care Nurse to join our team at Pan Am Clinic. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and contributing to the expansion of our healthcare services.
Key Responsibilities- Provide primary care services to patients in a variety of settings, including clinics and community areas.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
- Conduct physical examinations and provide nursing interventions as needed.
- Provide education and support to patients and their families to promote healthy living and disease prevention.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Graduate of an approved Registered Nursing education program.
- Minimum of two years of directly related experience in primary healthcare, community health nursing, or ambulatory care nursing.
- Experience with mental health counseling and diabetes or asthma education is an asset.
- Current registration with the College of Registered Nurses of Manitoba (CRNM) and possession of a valid Manitoba Class 5 driver's license.
- Proficient in computer software and applications, including Windows, word processing, and databases.
- Working knowledge of computers and keyboarding.
- Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
