Key Responsibilities- Active Participation in Patient Care Rounds
Participate in patient care rounds to provide pharmaceutical care services, including medication reconciliation upon admission and discharge, therapeutic drug monitoring, and other relevant services.
Clinical Expertise and Patient EducationCultivate and demonstrate clinical expertise tailored to specific patient populations, and provide valuable drug information and insights to physicians, nurses, and other healthcare professionals.
Medication Verification and SafetyMeticulously verify all medication orders to ensure accuracy and safety, and take corrective action where necessary.
Preceptorship and EducationServe as a preceptor for pharmacy students, nurturing their growth and development, and deliver patient education to promote optimal drug utilization and health outcomes.
Medication Reviews and Self-Medication ProgramsCoordinate and participate in quarterly medication reviews on designated units to enhance medication management, and initiate, instruct, and monitor patients in self-medication programs to foster self-care.
Committee Participation and Safety AwarenessActively participate in hospital committees as necessary, contributing to the institution's initiatives and policies, and maintain a vigilant awareness of conditions and measures that impact the safety of patients, residents, students, employees, volunteers, physicians, and visitors.
Availability and FlexibilityOffer Saturday coverage and share on-call responsibilities in rotation with other West Park pharmacists, ensuring pharmacy services are available when needed.
Requirements- Education and Certification
Bachelor's Degree in Pharmacy, Registered as a Part A Pharmacist with the Ontario College of Pharmacists, and 2 years' work experience in a healthcare setting.
Skills and QualificationsExcellent computer skills, designation certification an asset, such as geriatric pharmacist, anticoagulation pharmacist, antibiotic stewardship, diabetic educator, etc., and Doctor of Pharmacy (PharmD) preferred.
