We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.
Key Responsibilities- Patient Assessment and Care Planning: Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans.
- Service Coordination: Link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.
- Resource Management: Balance patient needs and choices with available resources, ensuring patients' values and preferences are respected.
- Education and Experience: Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.) and 2+ years of recent experience in community health or a related field.
- Skills and Knowledge: Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills, as well as knowledge of the healthcare delivery system and community resources.
- Personal Qualities: Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment.
- Experience with Diverse Patient Groups: Experience working with multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatric patient groups.
- Case Management Experience: Recent case management experience or related community experience.
- Language Skills: Ability to speak French or another second language.
