Personal Support Worker (PSW) Job Description Job Title: Personal Support Worker (PSW) About the Role

The Personal Support Worker (PSW) is a vital member of the inter-professional care team at the University Health Network (UHN). As a PSW, you will work collaboratively with healthcare professionals to provide high-quality patient care and support.

Key Responsibilities
  • Assist patients with activities of daily living, including feeding, bathing, skin care, oral hygiene, and toileting
  • Perform tasks delegated by a Registered Nurse (RN) or Registered Practical Nurse (RPN) in compliance with the Regulated Health Professions Act (RHPA)
  • Communicate and demonstrate basic information in accordance with a patient's pre-established Plan of Care
  • Ensure a safe environment for patients and staff by reporting and documenting unsafe conditions
  • Complete and maintain related patient records and progress notes, as per hospital policy
Requirements
  • Successful completion of a PSW program from an accredited community college
  • Current Basic Cardiac Life Support (BCLS) Certificate
  • Behavioral management training (such as PIECES, U-First, CPI) is considered an asset
  • Ability to handle daily mild to moderate physical exertion
  • Acute care and/or previous rehab experience considered an asset
