As a valued member of our team, you will play a lead role in providing high-quality patient care, working collaboratively with healthcare professionals to assess and determine patient care needs and eligibility. You will also provide access and referrals to community services, engage with patients, caregivers, and other healthcare practitioners, and work in a variety of settings, including our office, community, and healthcare facilities.
**Key Responsibilities:**
- Assess and determine patient care needs and eligibility
- Provide access and referrals to community services
- Engage with patients, caregivers, and other healthcare practitioners
- Work collaboratively with healthcare professionals to provide high-quality patient care
- Travel between sites in the Erie St. Clair region
**Requirements:**
- Registered Nurse, Nurse Practitioner (Current registration with the College of Nurses)
- Working knowledge of community resources and roles of healthcare professionals
- Good knowledge of Ontario Health atHome priorities, policies, practices, and service standards
- Good knowledge and understanding of legislation governing healthcare activities
- Knowledge of direct care/care coordination models used in community healthcare organizations
- Specialized education required in palliative pain and symptom management
- Valid Ontario Driver's License and access to a vehicle
- Ability to operate a computer
**Preferred Qualifications:**
- Excellent written and oral communication skills
- Strong interpersonal skills with the ability to provide mentorship and maintain professional relationships
- Demonstrated interpersonal communication skills and an oral/written proficiency with Canada's two official languages
