Salary: $36,400 per year

In this role, you will:

  • Arrange transportation for clients to and from home, utilizing local services and/or driving an agency vehicle when needed.
  • Support clients in understanding and implementing hospital discharge instructions, creating individualized plans of care based on agency assessments and client needs surveys.
  • Complete errands to meet clients' physical needs, such as nutrition, medication, and medical supplies, in accordance with established service plans.
  • Plan and arrange home management duties, such as cooking, cleaning, and laundry, in accordance with established service plans and in advance of discharge when identified.
  • Facilitate client and caregiver engagement, providing information on client-related issues surrounding transitions from hospital to home and/or prevention of hospital admission and readmission.
  • Communicate recommendations and support clients, caregivers, and those in the circle of care to address and repair safety concerns.
  • Provide companionship to older adults who suffer from social isolation.
  • Conduct risk analyses of clients' homes and evaluate their risk of falls using standardized risk assessment tools.
  • Support clients post-discharge to remove barriers and reduce risk in accessing resources and for follow-up discharge-related appointments.
  • Provide system navigation for clients and caregivers.
  • Document concerns, observations, and needs of clients in accordance with standards of documentation electronically.

Requirements:

  • Minimum of two years post-secondary education with a concentration in social services, gerontology, therapy assistance, or a related field, and a minimum of three years job-related experience or an equivalent combination of education and experience.
  • Previous knowledge of medical and technical terminology is an asset, but not required.
  • Fluency in English and French is required.
  • Excellent computer skills, including experience with Microsoft Office.
  • Knowledge of CIMS and other databases is considered an asset.
  • Valid driver's license and access to a reliable vehicle at all times.

Working Conditions:

  • The work is performed Monday to Friday in a combination of an office environment, hospital setting, in community, in agency vehicle, in own vehicle, and in client homes.
  • Travel between office and hospital, and other close branches.
  • Overnight stays for long-distance driving.
  • Requirement to work 35 hours per week with irregular hours without notice.
  • This job requires some physical bending, stretching, repetitive motions, and perhaps climbing, pushing a wheelchair with a beneficiary, helping them to get into and off the vehicle.
