POSITION DESCRIPTION:
The Clinical Practice Leader (CPL) plays a key role in leading clinical and professional practices, education, and research utilization.

As an agent of change, expert clinician, and collaborator, the Clinical Practice Leader contributes to enhancing patient care standards.

Reporting to the Director, Regional Nephrology & Diabetes, the CPL ensures the delivery of safe, high-quality patient care in Nephrology and Diabetes, encompassing in-centre hemodialysis and the Diabetes Education Program.


DUTIES AND RESPONSIBILITIES:
- Offers expert clinical consultation for patient care delivery
- Provides leadership in education, clinical documentation, projects, and committees within their area of expertise.

- Gathers, evaluates, and presents data on quality and process improvement metrics aligned with the Ontario Renal Network (ORN) and Lakeridge Health, and devises educational plans based on identified trends.

- Delivers education for additional skills and certifications beyond standard expectations and delegated controlled acts
- Leads or advises on the creation of organizational and program-based educational programs (including orientation and ongoing education)
- Plans and conducts Intra- and Interprofessional educational sessions
- Develops easily accessible educational opportunities to encourage staff engagement
- Utilizes various adult learning strategies and evaluation methods that align with the desired learning outcomes
- Relies on the best evidence available for all practice, educational, and professional leadership activities (e.g., practice projects, educational sessions, policy/procedure development)
- Participates in recruitment processes
- Supports staff in their personal and professional development endeavors.
- Assists managers in assessing staff performance as needed
- Conducts clinical practice assessments, offering educational support for performance issues and learning gaps as appropriate.
- Provides guidance on, or creates, performance improvement plans with staff as necessary
- Collaborates with human resources, occupational health, and management to ensure a safe work environment

QUALIFICATIONS:
- Active registration with the College of Nurses of Ontario
- Bachelor's degree in Nursing or a related health discipline;
- Preferably holds a Master's degree, or demonstrates progress towards one;
- Completion of Basic Cardiac Life Support (B.C.L.S) program;
- Mandatory certification in Nephrology
- Minimum of 3-5 years of recent nursing experience in a Nephrology setting, particularly with hemodialysis
- Theoretical and practical background in adult education, including program planning, development, and evaluation;
- Proven experience in program planning, evaluation, and managing organizational change initiatives;
- Demonstrated effective interpersonal and communication abilities;
- Strong leadership and collaboration skills;
- Demonstrated critical thinking, prioritization, and conflict resolution skills;
- Ability to commute between all locations;
- Familiarity with and dedication to Lakeridge Health's safety programs and practices, encompassing staff, patient, and environmental safety, and participation in regular education sessions on this subject;
- Consistent attendance record with the ability to maintain high standards;

- Compliance with confidentiality protocols;
- Must have a satisfactory performance history
