**Job Description**

**Key Responsibilities:**

• Conduct comprehensive assessments of patients' communication and swallowing abilities
• Develop and implement individualized treatment plans to address communication and swallowing disorders
• Collaborate with interprofessional care teams to provide patient-centered care
• Educate patients and caregivers on self-management and health promotion strategies
• Participate in quality improvement initiatives and research projects

**Qualifications:**

• Current registration with the College of Speech and Hearing Health Professionals of BC (CSHBC)
• Knowledge of speech language pathology theory and practice within a patient-centered model of care
• Ability to communicate effectively with patients, caregivers, and healthcare professionals
• Ability to work independently and collaboratively as part of a team
