**Registered Nurse Job Description**
We are seeking a skilled Registered Nurse to provide direct patient care in an acute inpatient setting, working collaboratively with an interprofessional care team to deliver person-centered care.
**Job Responsibilities:**
• Assess patients and collect information from various sources to make informed nursing diagnoses and develop effective care plans.
• Collaborate with patients and their families/caregivers to develop and implement care plans, promoting patient autonomy and dignity.
• Participate in care and discharge planning, ensuring comprehensive care delivery and effective communication with the interprofessional care team.
• Coordinate nursing care, prioritize tasks, and delegate responsibilities to support staff as needed.
• Collaborate with healthcare professionals to identify and resolve issues, adjust care plans, and address changes in patient condition.
• Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to support self-care and discharge planning.
• Document assessments, nursing diagnoses, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
**Qualifications:**
• Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• Ability to complete initial and ongoing patient assessment, provide nursing care, and communicate effectively with patients, families/caregivers, and healthcare professionals.
• Ability to promote person-centered care, integrate and evaluate data, and problem-solve effectively.
• Ability to teach and instruct patients and families/caregivers about essential health care topics.
• Ability to organize and prioritize work, collaborate with the interprofessional care team, and use relevant research-based evidence/evidence-informed practice.
